This Request must be completed by authorised persons or agents on behalf of the Owners Corporation. Council may reject the request if it is not completed by the authorised persons or agents on behalf of the Owners Corporation.

This Request may be forwarded to the Owners Corporation (strata schemes) or an Association (community schemes) in order to verify the details provided within this request if required for verification purposes. It is a crime to provide false or misleading information under the Crimes Act 1900.

By completing this request, you acknowledge:

By providing the password to other parties, you have authorised those parties to make bookings on behalf of the property. Council will not reinstate bulky waste collection services when the correct password is used to book a bulky waste collection service.
You have reviewed Council’s requirements prior to participating in the Bulky Waste Collection Service. The requirements are published on Council’s website and contained within the booking confirmation email, sent after a booking is completed.
